All agents who joined CWA prior to January 2010 are required to submit the FiMM mandatory Statutory Declaration (SD) for Continuous Eligibility Requirements of Unit Trust Consultants.
The deadline for the new SD submission is 30 April 2010. Agents who fail to submit by then shall be DE-REGISTERED FROM FIMM AND THEIR AGENCY AGREEMENTS WITH CWA WILL BE TERMINATED accordingly.
Consultants are required to fill up the following information in the new SD:
First Paragraph of the new SD
1. Fill in name as per IC
2. Identity Card number
3. Address in full
At the bottom right of the new SD
1. Agent’s signature
Please ensure there are no amendments on the SD. All amendments will need to be re-attested by the Commissioner for Oaths. Usage of correction pen is strictly not allowed.
